How to read it

Retrospective institutional account, not a June 1942 technical manual or individual weapon return. The 105-torpedo test belongs to mid-1943, and fault categories overlap. Its results are not a Midway failure rate. Later improvements and launch limits are not applied to 1942 weapons.
